General Description
Breast augmentation is one of the star surgeries in our country. It is one of the most requested and performed aesthetic treatments by Mexican women. According to ISAPS, during 2018, more than 70,165 of this type of surgeries were performed in Mexico.
What is breast augmentation?
Breast augmentation or augmentation mammoplasty is a cosmetic surgery performed to improve the size and shape of a woman's breast. The main reasons that lead a patient to undergo surgery are the following:
- Breasts too small for her figure
- Breasts that have been left empty or fallen after pregnancy and lactation
- Breasts that are different sizes
- Reconstructive Plastic Surgery after breast surgery
When performing an augmentation mammoplasty, the most important thing is to choose the size of the prostheses and the type of surgery that is going to be performed. The objective is to find the maximum balance between the wishes of the woman and a natural result.
How do I know if I am a candidate for breast augmentation?
In order to perform this procedure, you must be close to your ideal weight, that is, you must have a BMI of 30 or less. In addition, to undergo breast surgery you must be healthy both physically and mentally. If you have a heart condition, lung condition, or neurological disorder, you should discuss this with your surgeon as you may not be able to have your augmentation mammoplasty. If you plan to have surgery but in the future you want to get pregnant again, surgeons recommend waiting so that the results obtained are more lasting. In any case, it should be noted that this type of surgery does not put future pregnancies or lactations at risk. Tobacco is not recommended before or after augmentation surgery as nicotine impairs healing and wound healing. Therefore, if you are a smoker, you should quit at least 6 weeks before and after your surgery.
How do I choose my surgeon?
It is very important to choose an experienced plastic surgeon to ensure the success of the cosmetic operation. In Mexico, only plastic surgeons certified by the Mexican Council of Aesthetic and Reconstructive Plastic Surgery (CMCPER) are trained and endorsed to offer breast augmentation. You should check their credentials and their studies. We also recommend having different appointments before choosing your surgeon to compare and solve any doubts that may arise about the intervention, the pre and postoperative period.
It is important to review the following aspects before choosing a surgeon:
1. Check his studies and experience, make sure he is a certified plastic surgeon with an official degree in the specialty. To do this, check the page of the Mexican Council of Plastic, Aesthetic and Reconstructive Surgery.
2. Check that its facilities are correct, since it is a surgery, it must be performed in an operating room and must have the necessary resources to attend to any possible emergency situation. If you have questions about its facilities, you can contact COFEPRIS.
3. Make sure that it is the specialist who attends to you and not a commercial since you must be explored by a professional. Also do not get carried away by advertising with very cheap prices.
On the first date you should talk about all your doubts
How is the first appointment with the surgeon?
The first appointment with your surgeon is very important since normally, fundamental aspects about the breast you want to have and how the operation will be will be discussed. For this reason, at this visit the surgeon will examine the current size of your breasts as well as the shape they have. Another point that the professional will have to evaluate is the firmness of the skin, which can vary according to the age of the patient and her personal conditions. After this exploration of the breasts, it is likely that the surgeon will request a mammogram to see in more detail the state of the breasts, to see if there is any lump or suspicious mass, since cancer is one of the most frequent and deadly diseases in Mexico. Also, the doctor will do some preoperative studies that will be requested according to the age and physical condition of the patient. In this appointment the surgeon will also detail what techniques exist to perform a breast augmentation so that you have all the possible information about each one. Another important point that you should consider with him is the size and shape of your future breasts. Depending on these and other factors, the specialist will recommend one technique or another. As in any cosmetic operation, it is very important that you talk to your surgeon about your expectations, as they must be realistic. He asks all your questions and asks for photos of his results. For its part, it must warn you of the risks and possible complications of the operation. Some surgeons prefer that patients stay in after surgery. At this appointment take the opportunity to ask if you will have to stay one night at the clinic, what anesthesia will be used and the total price of the surgery. The plastic surgeon will also ask you if you are a smoker or if you are taking any medication, as these may affect the surgery and the postoperative period.
Types of implants or breast prostheses
Breast augmentation can be done with implants or through fat transfer. Breast implants exist in saline and silicone varieties, as well as many sizes, shapes, and textures.
- Saline implants are made of a physiological solution made up of water and salt that is used as a filler in the breast prosthesis. In the event that the implant leaks, the body will absorb the saline solution and eliminate it naturally.
- Silicone implants contain an elastic gel that can more closely resemble natural breast tissue. When placing them, the surgeon may need a longer incision. Surely, the patient will need regular check-ups because in the event of a leak, the silicone is not absorbed naturally.
- Cohesive gel implants are made with a silicone gel that is thicker than traditional silicone. They usually hold their shape better, but in return, they require a longer incision.
- B-Lite implants are innovative prostheses considered the lightest in the world. It is a cohesive gel silicone prosthesis but up to 30% lighter than the traditional one.
Techniques for breast augmentation
Breast implants can be saline and silicone. In addition, they can have different sizes, shapes and textures. The implants can be placed above, below or in a mid-position of the pectoralis muscle, depending on the area where the incision is made.
- Dual Plane: In the subfascial plane or position, the prosthesis is positioned in a virtual space created between the pectoral muscle and its fascia, which is a kind of thin cover that covers it. In this case, below the implant is the pectoral muscle and above, the muscular fascia and the gland.
In recent years, the subfascial technique has been consolidated since there is less risk of capsular contractures compared to implants located in the subglandular plane. Fat transfer involves harvesting fat cells from one area of the body, commonly the stomach, hips or thighs, and injecting them into another to achieve more volume in the area. The fat is removed from the body by liposuction, processed and purified by a centrifugation process, and injected, for example, into the breasts. With this technique up to 30% of fat can be reabsorbed depending on each patient.
How to choose the correct size for my breast augmentation?
Breast implants and fat used for fat transfer are measured in cubic centimeters (CCs). Keep in mind that the same implant size or amount of fat will look different on each person. It is important to discuss expectations and desired results with your surgeon to help you decide what will be best for you. Many doctors use digital imaging software as a way to see what breasts will look like with different levels of CCs. Another option to check the size of the prostheses is to try on a bra with them to get a rough idea of the volume. Some specialists recommend that if you like two sizes or measurements, "choose the larger of the two" because "the chest always falls somewhat and the view ends up getting used to it".
What is the profile and projection of the implant and why is it so important?
The projection of the implant will determine the sensation of size as well as its naturalness. This will be added to the one you already have before surgery, so the same projections may be different in different women. The degree to which your future chest will protrude from the thorax will be known as projection. The profiles or projections are measured according to their degree and there are normally 3 or 4 types for each prosthesis. Surgeons performing breast augmentation typically use the high or extra high profile. The medium profile is usually used for cases in which the patient requests a very subtle increase, that is, one that barely protrudes from the chest. You should consider that if you choose a higher profile you will have a feeling of a larger chest. This is due to the fact that it is projected more outwards and therefore, it is more attractive. Normally, surgeons recommend high or extra high projections for girls with almost no chest.
What type of prosthesis to choose: round or anatomical?
This doubt is one of the most frequent among women who want to have breast augmentation. Currently there are special meters so that the patient can see herself with the different volumes, having a much more accurate appreciation of how the implants would look in different sizes.
- Round prostheses: They are recommended when the patient's breasts have a harmonic shape and with more than half of the content of the gland in the lower pole.
- Anatomical prostheses: They are usually indicated when the chest is flattened or empty and with less than half of the contents of the gland in the lower pole.
It is important that above all, the result is natural and is consistent with the physical characteristics of the patient. The US National Library of Medicine National Institutes of Health emphasizes that the choice between round and anatomical prostheses is a very important decision in which the patient's anatomy, wishes and surgical history must always be taken into account. For this reason, the surgeon will indicate and recommend the ideal type of prosthesis for you and for the results you want to obtain.
Breast implant textures
If we classify breast implants according to their surface, it can be smooth or textured. When it comes to a smooth implant, its surface is completely smooth. In contrast, in the case of textured implants, the surface is rough.
- Smooth implants: They are the most used in most breast augmentation surgeries. These implants move more since, being smooth, they adhere less to the surrounding tissues. They are usually softer than textured ones, with less risk of rippling and cheaper. It should be noted that these implants have a higher risk of capsular contracture if they are placed above the muscle. Encapsulation occurs when scar tissue around the implant grows and therefore presses on the implant. This makes the chest hard and requires a change of prosthesis.
- Textured implants: Having a rougher surface, they better adhere to the surrounding tissue and there is less risk of capsular contracture, although on the other hand, they may have a higher risk of rippling. These implants are usually more expensive than smooth ones. become hard and a change of prosthesis is needed.
It is important to speak with the surgeon so that he can explain the advantages and disadvantages of each implant and agree on the most appropriate one.
The surgery usually lasts between one and three hours.
How is the day before and the day of surgery?
During the different appointments, the surgeon will give you all the information regarding the preparations for the surgery. In the hours before the operation, he will tell you what foods and liquids you can eat and for how long. Remember that if you are a smoker you must quit tobacco 6 weeks before. If you take any medication, the surgeon will tell you if it can affect the operation or the postoperative period and therefore, you must stop it. We recommend that you be accompanied by a family member or friend on the day of surgery. In addition, it will be good for you to have someone help you, especially during the first days after the intervention. Breast augmentation surgery usually takes between one and three hours. After you arrive at the hospital or operating room, the anesthesiologist will administer anesthesia. General anesthesia is the most common, but local anesthesia with sedation is also common. After the procedure is complete, you'll be taken to a recovery area for a short period of observation. Unless your doctor recommends otherwise, you can go home with the bandages and your surgical bra that same day or the next day, it is already the preference of each doctor and what has been agreed upon.
How long does breast augmentation recovery last?
In the hours and days following the intervention, it is normal to notice discomfort in the breasts, that they are swollen and that bruises appear in the treated area. Also, the area may be very sensitive. After 48 hours, body temperature may increase and small bleeding may occur. After approximately 3 days, the bandage will be removed and replaced by a post-surgical bra that must be worn day and night for a month. The first week it is best not to raise your arms and carry them practically close to your body. During the first month the patient should sleep on her back and after that, she can start sleeping on her side. Until after three months it is not advisable to sleep on your stomach. Remember that you must follow all the recommendations that the surgeon indicates for the postoperative period. In order to promote proper healing after the intervention, the following is recommended:
- Take all medications that the plastic surgeon has prescribed
- During the 15 days following the operation do not take anticoagulants
- Do not make physical effort or exercise and try not to move your arms a lot
- Wear the bandages and the post-surgical bra for as long as the surgeon indicates
- Do not smoke or drink alcohol
- Do not touch the bandages or remove them
- Sleep in a position that is on your back
- Take care of scars with products that help heal, such as rosehip oil.
One month after the operation, you can start doing light sports that do not involve using your arms. In addition, special care should be taken with the sun since the breasts should not be exposed to the sun for six weeks after the operation. In about three months the recovery is complete and the patient can now carry out all the activities that she did before the augmentation mammoplasty.
How long does results last?
In most cases, the results last a long time. However, it is important to monitor the condition of the implants regularly. Those with saline implants can check their status simply by looking in the mirror. Leakage will be obvious as a ruptured implant will deflate and the saline solution will be absorbed into the body. The FDA recommends that patients with silicone implants have an MRI three years after initial placement, and every two years thereafter. If you choose to undergo fat transfer breast augmentation, you may lose some volume over time. Your breasts can also change due to pregnancy, weight fluctuations, hormonal factors, or aging.
Results
Patients who have undergone breast augmentation are usually very happy with the results obtained. In addition, thanks to the regular reviews that will be carried out with the plastic surgeon, it will be possible to see the evolution and observe if there are any complications. If appropriate due to the patient's age, mammograms will also be performed to detect any possible anomaly in time. Sometimes there are women who request another operation because they are not satisfied with the result because the breasts are too small or too large. To ensure that the size of your breasts is what you imagined, it is important to have no doubts about the type of implants that the surgeon and the CCs are going to place. These points are the ones that most often worry patients before the operation.
